Patient Information

We welcome new and returning patients to our practice. For questions, or to schedule an appointment, call (719) 539-4600. You can schedule your appointment at either our Salida or Gunnison location.

At Your Appointment

Please plan to arrive at our clinic 15 minutes before your scheduled appointment time. Both new and returning patients will need this time to check in.

Please note our late policy: If you are 15 minutes late to your appointment, you will need to reschedule your appointment. Out of courtesy to all of our patients, we must enforce this policy.

About Insurance

Patients must bring their insurance card to their appointment. If you do not provide us with insurance information, you will be considered a "self-pay" patient for the visit and payment will be expected at your appointment.

Payment Policy

If you do not have insurance, or if you have an insurance plan with which we do not participate, payment in full is expected at the time of your visit. Co-pays will also need to be paid at the time of the visit. For your convenience, we may be able to bill to your insurance company for you. However, you remain responsible for payment of the bill regardless of your insurance status. We accept payment in the form of cash, check and credit card.

Your Medical Information

All patients should provide us with information regarding current medication, including the drug name and dosage. If your medical information changes between visits, let us know so that we may update your records.

Cancellation Policy

Our office requires 24 hours notice to cancel an appointment. If less than 1 full working day's notice is provided, a cancellation fee of $50 will be added to your account.
