Patient Information: New Patients

All patients, whether new or returning, should plan to arrive at our clinic 15 minutes before the scheduled appointment time. Both new and returning patients will need this time to check in.

For your first visit, you should allow extra time to complete initial paperwork, including a medical history form. If you prefer, you may download the forms below, fill them out ahead of time, and bring them with you to your first appointment.

Please note our late policy: If you are 15 minutes late to your appointment, you will need to reschedule your appointment. Out of courtesy to all of our patients, we must enforce this policy.

New Patient Forms

All patients should provide us with information regarding current medication, including the drug name and dosage. Please also bring information regarding your insurance coverage. If any of this information changes between visits, let us know so that we may update your records.

Additional forms are available on our Download Forms web page.

Payment Policy

If you do not have insurance, or if you have an insurance plan with which we do not participate, payment in full is expected at the time of your visit. However, you remain responsible for payment of the bill regardless of your insurance status. We accept payment in the form of cash, check and credit card.
